Our recent survey experience with CHAP. Very strict! Looked in every nook and area including the supplies for expirations. What I took away from the survey: If you think that you are ready, check again (have another person look over the charts) Remove the 90 day evaluation of your staff from your policy and procedure(s), unless it is required in your state. Do not trust the Policy and Procedure Manual handbooks! Look and be sure that Advance Directives and Patient Bill of Rights information is there. If not, include it with your admission consents and paperwork. Last but not least, never let them see you sweat! What is your experience?

Did you know that the Comprehensive Assessment is one of the most commonly cited deficiencies for hospices during certification surveys? 418.54 L520 Four initial assessments must be completed during the first five days of care, regardless of acceptance of the services by those disciplines. RN Initial Assessment, SW Initial Assessment, Spiritual Initial Assessment, Initial Bereavement Assessment. What are you doing to meet this standard?
